I don't believe it's possible to alter the server-side behavior of items. Also, that's not even an item that they ever had in their inventory. You(As a newb) select 'Use' on a Heart Sticker, type in a name, and a DIFFERENT item is put into that person's mailbox, and the item you used is consumed. The different colors and behavior are proof.

Technically speaking, if someone really wanted to do some malicious to you they could spam 'Heart Sticker send' packet, flood your mailbox, and make you unable to open it at all. But that's the only thing they can do.

Maybe a hacker'll go hack an NPC so that when you go talk to it, it gives you an item that immediately crashes you and makes your character unable to load.
