I've had teachers that did 3 out of those 4 things.
One of my math teachers was the one who assigned homework every day, and yet mostly checked if you just did it - then occasionally "surprise" us by checking your actual percentage on them.

My most hated item on the "Back to School" list? 3-ring binders.
I end up buying them, and I never use the actual rings - and used so so much space, that my back hurt from the weight. So I just used an accordion binder for the past ~3 years.

Teachers that do this infuriate me more than I can aptly express in words. I will argue stupid test questions to the bitter end out of a mixture of spite for my idiotic teachers, combined with a healthy dose of "How could you possibly have ****ed it up this badly?".
in middle school i had a teacher who did WEEKLY binder checks, and because of that, my final grade was around a c average. i was unorganized back then (still am) and at one point i got pissed at her because she was being a HUGE B*TCH and telling me i need to stop being lazy and actually do her work, where i slammed the book on the ground and left.
was i lazy? nope :I i did ALL of her work, i just could never remember to put it in my binder. and she had the nerve to say that to me.
If your teacher isn't doing their job, it's time to report their ass and get them out of the building.

Yup. Got my 7th grade Math teacher fired because she was a useless, psychotic and generally abusive *****.
I had a Spanish teacher that did notebook checks last year. She collected them every quarter, and they were a big part of our grade. So, I turned my notebook in one quarter, and a week later she tells me I never turned it in. Even with my classmate as a witness to me turning it in, she still claimed I never turned it in. So because she lost it, I got a crappy grade that quarter. Also, I heard of the same thing happening to one of my classmates from Biology. We never reported it though.
I had a teacher that sort of did the top 2. The grade wasn't that dependent on binder checks though, and she didn't give us that many handouts that we had to hole punch ourselves (with a hole puncher she provided).
I use 3-ring binders because it's pretty easy for me to keep track of things that way, and because I only need 1 for the whole year, and I can keep re-using it every year. *_*
I am sorry to you all - I have been fortunate enough to only have that happen for one or two school years

Also, I f*cking love you for posting these.
Personally knowing how much work teachers receive, it is easy to lose and forget one assignment out of 100+ more.
Next time, it doesn't hurt to have them sign a little piece of paper saying that you did turn it in - if not anything else, report it if they keep denying that it could have been lost.
