Why does starpaw sound like he has lvl2 access then?

Because either you misread his/her post (most likely) or he/she got lucky enough to get a Lv 2 reforge on an accessory in the same way that someone would be lucky enough to randomly get an elemental Lv 4 weapon.

I'll just ask him I guess, but it does sound like it's capped at 2 and not 1. Unless he somehow got 2 r2 accessories with what he wanted? (That's the way it sounds to me)

Accessories are capped at Lv 5, not 2 and not 1.

However, you will NOT see an accessory higher than Lv 1 in the same way that you will not see a Lv 4 Lightning weapon with only NPC reforges. The exception is not the norm, it's the random luck from someone who won the lottery after being attacked by a shark while being struck by thunder in the lost city of atlantis.

Though that aside, I'm pretty sure the person just got a pair of Lv 1 Blacksmith accessories after spending a couple hundred K on them.

Hm..you're right. My bad then, I guess..the post sounded as if he had gotten lvl2 to me.
