Reviving someone using Phoenix Feathers is all you need to do, I believe.
The amount of Seals you have shouldn't matter...

It's not working for me either, but I thought it was because I was using trainee feathers.
I've used some regular feathers since, but I haven't checked to see if it worked.

But I believe you have to be reviving during the time you have the quest, not before it since the title is unavailable to other classes and you only get notified of its existence once you accept the quest.

Yeah need to revive while you have the quest. You can revive 9001 people before you get the quest and still not get the title.
