so I've been wondering for a while now but are midbosses completely random or do they have some sort of trigger?

From what I've seen, it's the flip of the coin. However, you can tell early on which midboss you'd be fighting depending on the random map you load into.
An easy example would be in the 'Revenge' battlequest, which contains two minibosses, Bryialne, and Embermane with Shadow Fang paired.
In Bryialne's 'map' we would load into the area where right off the bat we encounter the two revolving spike traps, if we go into this map, I think it's guaranteed that Bryialne will show up right before the transitional door, this will be underlined if you encounter a large number of wisps as you approach the last room.
In Embermane and Shadow Fang's 'map', while I can't place a finger on the beginning, it's the map where you encounter the large spike trap. You know, the one where you gotta hide out in the alcoves for it to pass. Once you get past that trap, there is a small hallway, and you'd be greeted by the boss music just before you see the pair.
Note that not all battles containing midbosses have a different map, and will in fact share a map. So it is possible you won't really know which boss will appear even if you know that a particular map has a boss coming up, such as in decisive battle, where Bryialne and Mad Grizzlepaw share the same map, and will spawn in the same room. The only way you can tell which one will spawn before they actually spawn, are the mobs that appear before they do so. (Wisps for Bryialne, and more Gnolls for Mad Grizzlepaw)
In conclusion, I believe that it is random, but I think that in most maps, there are telltale signs which can give you hints of which miniboss you'd be facing. There are also some battles that have a chance of spawning a miniboss where you wouldn't face a miniboss at all, which an example of would be decisive battle, where you have a chance of fighting Mad Grizzlepaw, Bryialne, or nothing at all.
Sooo, to answer your question, it's all chance, but you can better prepare yourself once you've run the battle a number of times to find out what spawns where in each particular battle, coupled with the random rooms.

P.S. For clarity's sake, when I'm talking about the maps pertaining to their respective bosses, I'm talking about the mini area with it's own loading screen that would contain the miniboss. There is no possible way for you to find out who you'll be facing RIGHT at the beginning of the battle, unless of course that beginning map will contain a midboss.

I believe in "decisive battle", you can still see if Bryialne is spawning or not. At the end of the map before Bryialne's stage, you will come into this area with a few tentacle monster/gnoll/spider. There is a hidden jar behind the wall to the left. If no wisp spawn when you go near and break it, Bryialne will spawn in the following map.
Edit: so I think it's more likely that the maps are determined in the beginning, instead of generating right as you progress.

I'll test that out, though in all my times running through, that erg (and any erg, for that matter) would spawn wisps whether or not Bryialne would pay a visit.

Each boss has its own map that it will spawn in. In some stages, there are certain map sequences that will appear, in other stages, the boss map gets injected into the pre-determined level layout. While I'm not exactly familiar with all of them, I can give a few examples. Western Passage is one that always has the same few maps, whether or not Eflame spawns. The only difference is, if Eflame DOES spawn, an extra map is injected right before the boss map. On the other hand, if you look at the Decisive Battle variations, there's two distinct patterns, one will always lead up to Bryialne, and the other will always lead up to Mad Grizzlepaw. If you encounter the spider pit map, Bryialne will spawn, but if you encounter the Giant Roller map, then Mad Grizzlepaw will spawn.

Then there's a weird one, like in Kobold Chief, which is only triggered if you're under the bridges, and you happen to stand right in the middle. If you do, Loud Ekublah spawns. If you make a sprint for the ladders, or spear the bridge-breakers, you can avoid him completely.
