January 16, 2011 1PM-2PM(EST)
I have withdrawn my Tail Cap from my pet's inventory and started to do Math Dungeon and until halfway the character screen froze. I forcefully exited through task manager and checked online that Channel 2 was starting to suffer a latency problem and I couldn't relog in, because my character was still online and not properly disconnected until around 7-8PM(EST).
January 16, 2011 7PM-8PM(EST)
I logged in after the unscheduled maintenance for the Tarlach server, and checked that I was roll backed to where I was at 11AM-12PM. I began to run Math Dungeon again and withdraw my Tail Cap from my pet's inventory and found that it was not there. I guessed that the rollback affected characters, but not their pets.
After an extensive investigation, we were able to verify the loss of your items. At this time, these items have been mailed to your character's in-game mailbox. Please be aware that these items are basic items of random color, as unfortunately we cannot return you're the exact item with stats and/or enchants to you.

In other words, I submitted a ticket to get back a 1Mil item and in reply for an item that's worth barely 25K. Yeah I got owned by Nexon.

It's their policy... Which is a very stupid one for this particular game, considering that an item's worth is largely determined by its enchants and colors.
I lost two Light Lilly accessories in a crash some time ago, took me around 3m+ to make both and I got the same response... Others have lost items far more expensive than that and none have ever gotten anything back, besides worthless unenchanted replicas of a random color.
Thankfully, Alexina hasn't had that type of crashes in quite some time... but after that happened I've been so paranoid that none of my hard-earned equipment has ever left my personal inventory since.

This is how items are saved in inventories:
1. The game has these things called "ticks" that consist of 5 real-life minutes.Every time these things happen, you lose some true dura, food takes effect, egos get hungrier if they're out, locations are saved, and inventories are saved, among other things I probably forgot.
2. Logging out saves inventories, even if you haven't been on for five minutes. Changing channels works, also. I hear changing maps saves inventories too, but I can't confirm that for sure so I won't list it as a sure bet.
3. When you de-summon a pet, its inventory is saved. However, your inv isn't saved until you've hit the next "tick", so if you crash, and a tick hasn't passed, your item wasn't saved in YOUR inv or your pets.
Recommendation?
Every time you move something you're attached to,log out or change channels. We shouldn't have to, but stuff like this happens sometimes. x.x

In my own experience with getting rolled back -has gotten rolled back probably a dozen+ times-, the game does not save every durability tick.
Putting something in a pet and desummoning it doesn't necessarily work either if anyone is thinking of doing that, I've lost the items before from doing that and having the item disappear into nothingness after rollback.
Bank works though, there are a few people with multiple Brionacs on Mari...
My recommendation is the same as Adelynn's, cc or relog after equiping something precious. Servers crash far more just before and after major update too.

I don't think it's right that they "can't" give you the right items back after they messed up. You look at all the fancy effects that GM's can have on their characters, and you would think that they would be able to send you an item with the right color(s) and/or enchant(s).

I've yet to actually see that happen but I guess it can be a possibility. I wouldn't recommend trying it though, because the only super safe ways that I know of saving an item is logging out or changing channels.

Nope they put in a safegaurd against duping like that. You lose the item.

There's no excuse for them to not be able to restore color and enchant as long as it's reported within a reasonable time frame for logs of the item to exist.
If such data isn't logged then their system is worst than I even though.
I suspect it's just lazyness.

Oh, yeah, good thread. I lost my Wooden Needle a week back because of a force logout. Sent a ticket, got a stock one in mailbox. It's ridiculous.

It is not that they are incapable of creating a brand new item in any color or enchant they want. They've proven numerous times over that they already have the ability to spawn anything form mobs to items.
The policy is because investigating the truth of the claim requires a lot more work. They would need to store backup saves of your account data in order to be able to "roll you back" as it where and restore stuff. From what I can figure, Nexon/devcat is conserving resources by limiting the amount of data that needs to be saved per user. I imagine there is only one "save file" for each of your characters. Pets are separate characters, so when one is desummoned, it gets saved in it's own file. Your character has it's own save file. When you experience the crash... there is only one saved data to go to for you when you log back on... the one that didn't have the item you pulled off it.
An extra backup per character would double the amount of date needed to be stored per user. And might not even be enough to prove a player even had an item. Lets say the pet's backup data had the enchanted tailcap in it. Sure, that would prove that between the current and backup saves you owned and somehow lost a tailcap with x enchant and y colors. But it wouldn't tell how you lost it unless there was also a saved record of every transaction your character did as well. Did you maybe trade the tailcap away, mail it, or drop it on the ground? Even more info that has to be saved. And possibly investigated in situations like this.
My guess is that the GM's don't actually investigate anything at all. If someone tells them they lost a tailcap due to the crash, they probably just confirm "yup, there was a crash, and people report losing stuff in these crashes. here, have a tailcap. derp." I wouldn't be surprised if someone could report the loss of something much more valuable (say some gacha dress or other fashion item) even if they lost nothing at all, and have a GM say "here ya go. derp."

I was on maple last night and ch 10 crashed and I lost my exp capsules and went back a level >_>
