If you buy perma undies, are you still able to wear the original undies or other perma undies while being able to switch around any time you like?

Yeah, you can use anything you buy from the cash shop as long as it has time on it before it expires.
And when you have nothing on you just use the default stuff.

Yeah, they're just items that take up a slot like a piece of armor would, you can swap them out with other "armors" - the slots are just in the "Cash" tab of your inventory.
