Ghost Hunt is pretty good for horror. Watch the 2nd arc late at night in a dark room =D

Higurashi no naku koro ni at the top of the list. Period. Its fantastic.
Followed by its sequel series, higurashi no naku koro ni kai and higurashi no naku koro ni rei.
Also with the same writers come Umineko no Naku Koro ni, but the anime adaptation isn't as good as higurashi was.
For gore, there is Elfen Lied. But after you watch the anime I suggest you read the manga. The story splits off at the last episode and ends the series half way through, so you can get on the manga to really see where it left off.
Chaos;Head is pretty good.
Deadman Wonderland is only just rolling out in anime. I haven't seen it but the manga is decent. If you're really looking for a good action packed gore/horror manga, Mirai Nikki takes the cake though.
Alternatively there is a light humor gore/horror manga Franken Fran, which is pretty fantastic.
Highschool of the dead has not sparked my interests despite my obsession with zombies and boobs. Read the entire manga, and was thoroughly disappointed.

Those remain to be my favorites.
And there's going to be another OVA for Higurashi this summer!

This. I mean, it's watchable if you don't ever touch the source material, but it's impossible to solve the mystery by watching the anime. Though if you're just watching it for the gore department, the anime did a pretty decent job on this iirc.
Here's a pretty popular amv I recall when thinking of Higurashi. It pretty much sums up the mood of the anime quite well. Very spoilerrific though.

It's a great combination between psychological horror and standard horror only offset by it's not-good animation.
Others include Mahou Shoujo Madoka Magica, Serial Experiments lain, and maybe Kara no Kyoukai, but that's more of a psychological horror than an actual horror. So, it's only creepy if you really think about it.

He makes a lot of disturbing mangas. Those most of them are "gross" rather than scary. Uzumaki was probably the best.
If you're heading straight for gore, I've seen series that put every one of the animes and manga mentioned in this thread down to a 4 on a scale of 10. But its so sick and violent in nature I'm sure I'll have complaints if I talk about it here.

Berserk, Claymore, Akira, Spriggan, Ghost in the Shell, hellsing ova, Baki for gore.
Lots of stuff I haven't watched like Vampire Hunter D, Kite, Mezzo? (hentai but gory)
If you only like moe stuff like Paul, you're not going to enjoy any of these.
