It would probably depend largely on your location, but the fact that they're using the word does mean that they are insensitive to homosexuals. If someone's gay and someone else has problem with it, they would insult with the word "gay," to describe the person, followed by conventional curse words <.<

They aren't just visual, no. They're very socially denotative terms. There's an entire subset of negative stereotypes associated with them.
And yes, people do find f----t offensive in that way, and it is also at times still used that way.

I used to say "Gay" a lot, in a negative way. It wasn't that I disliked homosexuals, but in middle school, everyone said it. It became a really bad habit. I finally managed to stop saying it last year.
When it comes to derogatory words, it really doesn't matter what you mean. It is insulting to some people, and that alone is enough reason to stop saying it, in my opinion.
