Which? Co-dependency doesn't go away very easily, though it kind of depends. If it's something like an abusive relationship, then it usually goes away eventually if you're lucky. But when it's with a family member or friend it tends to last longer. I'm 99.99% sure I'll have a co-dependent relationship with my Mom until one of us dies.
The only example of co-dependency that I can think of besides my own would be the Winchester bros from Supernatural. But the most common that I've heard of is, as previously stated, abusive relationships. Or just relationships that are unhealthy in general. Though I think that co-dependency isn't really that bad if it doesn't fall into the abusive/unhealthy category. It just means you rely on the other person to the extreme and in most cases, literally cannot live without them. I figure it's most common amongst family when you've been through a lot of shit/were always close.
