The Santa Casa de Itapeva hospital has confirmed the death and a post-mortem examination will take place on Monday.

Actually this is really horrible. Did the actor not struggle in a the few seconds before he lost consciousness? I'm surprised it took as long as it did to see the problem. There really should have been a spotter watching him and the supporting ropes.

Well, even if there's no difference in behavior between one deliberately hanging themselves compared to one doing so by accident, there still should have been someone monitoring him and the support structure. But then, it's easy to say that in hindsight.
I'm still curious though why it took 4 minutes to realize there was a problem. I don't know how the play goes (as I've never seen such a play), but I would guess the actor should have exited the stage (if this went as planned) some time before 4 minutes passed.
