I mean, there's a pretty big gap between "I'm really tired, I should've gone to bed 10 hours ago" and death.
Unfortunately, its not something you just do on the spot. Basically, he "trained" for this. By living a lifestyle of extended lengths of gaming, he was making himself more and more used to it, and eventually stretched into dangerous levels while not being able to recognize those dangers.
That said, I'm sure he was tired and hungry, but he probably didn't even mind them. Malnutrition and lack of sleep are not the cause of death in these scenarios, but blood clots. Blood clots naturally occur for everyone if they sit still for long periods of time. Just small amounts that clear themselves when we stretch or walk a bit. If a clot forms in a bad place, and you make a sudden movement in your body to propel the blood elsewhere, you might form serious complications.
Even at 40 hours of gaming, if he had just gotten up to stretch every 5 hours, he would've been just fine, if not a little malnourished and fatigued.
Edit: The healthy recommendation is actually stretching once every hour to reduce the stress on your body from sitting still.
