I had something similar happen in a bandit homestead, but instead of being in a homestead shadow tara's backdrop and music began playing, it looked similar to what is pictured there.

Earlier today, I went into a Provocation run and the mission like, failed to register. Nothing spawned, not even barrier spikes or an orb, the shadow mission information, timer, and exit button didn't appear, the quest didn't appear in the quest log, and I could even leave my party, inside the shadow realm.
