It's from the crown. The person who took the screenshot could be using mods, but the crown does turn people blue.

One of the biggest reasons I wanted an Ice Crown is because it turns your skintone blue.
Seeing how so much of you guys hated "Ice Crown" just because of blue skintone... I'll be glad to buy one for lower price.
