Well, depends how up to date.
Usually there is 1 cause, being a certain place, a certain item, or just other characters.
Find the cause, and update!

Take, for example, my house key. Sometimes I crash, sometimes I dont.

Also, the mod system you chose on using mods much more than the pack files, and goes into slightly more extensive mods, so not many people besides the creator would know.
Either choose a different modding system, or update, as I said.
