Last I checked there's a number of people doing it on the official forums, you could check the threads there for a rough idea on how it goes. In general pricing varies based on how long it takes the artist to do and how much people will pay (can usually get suggestions if you post examples I guess).

What'd you do? Generally you upload the image to some image hosting site and then provide one of the links it gives.
