Well, how I do it is:
After a perfect guard, release the guard immediately and then spam the smash button.

To counter, you don't have to time it exactly with a Perfect Guard.
That said, you have to smash during the Guarding animation.

Also, it doesn't HAVE to be a perfect guard. You can counter off a normal guard.
I tend to spam the smash key after letting go of guard like most people already said.

You are saying that is the mistake, and you shouldn't release?
Or you should release and follow with a smash, as you stated?
