There's a huge debate about the pragarah itself on the wiki and on the korean servers, it's a very gray area.

Morgant said he took a part in Elatha's creation. Some people take this to mean he is his father, it doesn't have to be. It could very well be a god father, or he could have performed the surgery, he could have cut the chord, he could have taken care of Neamhain. It's foolish to say "Yes, he is for sure the father" or "Yes, it means he cut the chord" if you don't know for sure. So sorry, can't really help on that hole. =\

If you meant the initial revival, I think it was explained in the underwaterway sidestory (or maybe G12?) that when the Corrupt Alchemists revived Claimh Solas (and supposedly did a better job than Cichol, Fallon, and crew), his cry reawakened Nuadha.
If you mean how is Nuadha back after G12, I don't think we actually kill him in G12 do we?
As far as I know no explanation was given, but I have yet to complete Saga 2. The most likely explanation is that Cessair's Heart became her vessel, because Cessair's Heart was in Renes but all of a sudden poofed until after all the shit went on, and because Macha looks like Cessair's Heart. And as much as I'd like that explanation of their resemblance to be true, there is no evidence behind it. I'm also still unclear on how it happened, was Tarlach TRYING to call up Macha?
A literal plot hole would be that giant hole at the end of G1. They never explained how it was fixed or even if it was fixed at all.
Or maybe i skipped too much dialogue
